OBSERVATION INDEX
Describe the symptom without naming the cause
The same visible change can have more than one explanation. Accurate notes are more useful than an online guess.
Write what happened
Use plain details such as 'the bedroom door rubs at the top' or 'the hallway floor feels different near the seam.' Avoid turning an observation into a repair instruction.
Add a date and location
A rough date and specific room help the provider follow the pattern. Include relevant recent plumbing, skirting, appliance, or repair work when known.
Escalate immediate hazards
Gas odors, electrical hazards, active leaks, or an immediate safety concern call for the appropriate emergency or licensed professional. Do not wait for a referral form.
